Circumstances change in life & I was wondering if any have had to give up seeing their transplant team & how that works.  Maybe having to move away from the area or otherwise made it necessary.  What are the hospitals' policies & requirements for this situation?  Thank you for any thoughts you can give in this regard!

  • Normally you would have arranged to be followed by a Tx center, near where you relocated and that is recommended by your Tx team. They would assist with the transfer of records and provide information to keep from having a break in coverage. Even if you have already relocated, this valuable resource will be most helpful with the transition. It all begins with a call to your previous Tx coordinator. Good luck!
